We accept a wide range of commercial medical plans and strongly encourage patients to contact their insurance company directly to confirm our in-network status due to "narrow network plans" we are excluded from with these companies. We do participate with Medicare. Additionally, we do participate with select Medicaid programs for established patients only.
HMO plans - you are REQUIRED to have listed a PCP from our office prior to being seen. Please contact us to confirm you choose the correct PCP for your plan. If your PCP is not listed as one of our providers on your service date(s) - you may be responsible for the full cost of your visit.
HMO plans often require insurance referrals (not just our provider referring you, but actually letting your insurance company know you are being referred to a specialist). Please allow up to 5 business days for non-urgent referrals to be processed.
We accept cash, checks, and credit cards (Visa, MasterCard, Discover & American Express). Copays and balances are expected on the date you are in the office. Please have your payment ready at the time of check-in/check-out; this saves you time and postage, and helps keep our billing expenses down, so we do not have to eventually pass that cost on to you.
If, for any reason, you cannot keep a scheduled appointment or will be delayed, please call us as soon as possible.
Effective, January 1, 2023 - we reserve the right to apply a $50 no-show fee to your account for missed appointments without a 24-hour notice.
Automated reminder emails, calls, and texts are a courtesy and should not be utilized as the only means of remembering an appointment.
